Our Services
The physician staff is comprised of three pediatricians, two physicians who are trained in both pediatrics and internal medicine, and one general internist. In addition to the physicians, an adult nurse practitioner provides services in our office. Pediatricians are trained to provide comprehensive care for children, including preventative care. Children often present different symptoms than adults; they can also have different diseases. They may need different prescriptions or treatments than adults. Pediatricians are uniquely trained to recognize the importance of these differences with adolescents, young children, and newborns. An Internal Medicine physician (also called an "Internist") specializes in the primary care of adults. An Internist's training is both broad and deep. They are equipped to deal with whatever problem the patient brings; no matter how common or rare, or how simple or complex. Internists care for the whole patientdisease prevention, promotion of health, as well as puzzling, chronic and multiple illnesses. Scheduling or Canceling an Appointment We make every effort to see you at your scheduled appointment time. To assist us, it is very important that you arrive on-time for your appointment. Patients arriving late, may be asked to reschedule. If you need to cancel your appointment, please give our office at least 24-hours notice so that another patient can be seen. Failing to show up for an appointment, without proper notification, more than two times in a 12-month period may result in the termination of our physician-patient relationship. If you would like, in advance to your visit, download and complete the following required forms.

Prescription Refills Prescription Refills need to be called-in during normal office hours so that your physician can access your medical records. Some prescription refills may require an office visit. You may request a refill by leaving a message on our Prescription Refill Line at (765) 966 - 5527 or toll free (800) 738-6330. Please leave the patients name, parent's name (if patient is a minor), physician's name, name of medication, strength, frequency taken, and your pharmacy's name. Please give our office 24-hours to process your refill request. Completion of Forms/Letters Requests for the completion of forms (i.e. disability, Medical Leave Act, school physicals, etc) or special letters require at least one week notice. Many forms will require a $10 fee to be paid at the time of completion. Bear in mind, some forms may request an office visit. Billing & Insurance Pediatric & Internal Medicine Center participates in several area insurance networks, as well as Medicare, Medicaid, and the Hoosier Healthwise program. As a benefit to our patients, we submit claims to most insurance companies. Patients are expected to present insurance card at each visit and failure to do so may necessitate rescheduling of an appointment. Annually, we have our patients update their patient registration forms. However, if your insurance name or address changes at any time, please notify our receptionist.
Pediatric patients monthly billing statements will go to the address of the custodial parent. Patient Services
